High-fat, low-carbohydrate ketogenic diets have been used for almost a century for the treatment of epilepsy. Used traditionally for the treatment of refractory pediatric epilepsies, in recent years the use of ketogenic diets has experienced a revival to include the treatment of adulthood epilepsies as well as conditions ranging from autism to chronic pain and cancer. Despite the ability of ketogenic diets therapy to suppress seizures refractory to antiepileptic drugs and reports of lasting seizure freedom, the underlying mechanisms are poorly understood. In this interview we explores new insights into mechanisms mobilized by the ketogenic diet.

SHR # 2504 :: New Insights Into the Mechanisms of the Ketogenic Diet
Show Notes:
[6:03] The 100th anniversary of the keto diet inspired this research.
- Ketone bodies were discovered 20-30 years after the diet came about.
[9:20] Are all of the benefits due to the production of ketones?
- It is all of the different mechanisms that are involved in producing ketones.
- E: ketone product adenosine production methylation.
[12:39] Do ketone levels have to be >2 mmol to get therapeutic benefits?
- The human body is designed to adapt to the fasting process.
[15:05] Are there any benefits to exogenous ketone salts or esters?
- It depends on caloric balance and carbohydrates available in the body.
[17:35] Ketosis vs. Ketolysis.
[19:05] How does ketosis affect sleep?
- Carbohydrates around bedtime can interfere with sleep.
[23:08] why do exogenous ketones raise insulin when ingested?
- Any excess in nutrients can result in conversion to sugars.
[24:25] Why is a keto diet useful for bipolar and other mental disorders?
[33:49] Would a ketogenic diet re-sensitize insulin receptors?
- It could prove to be very useful for Type I and Type II diabetics.
[36:49] Red meat is okay in low amounts.
- High levels of amino acids can be converted into sugars.
[39:10] A blood test for ketones is the most accurate way to measure ketones.
[40:12] what other mechanisms were involved with ketosis?
- Decanoic acid dampens neuronal excitability.
[42:45] Saturated fat: is it harmful to the ketogenic diet?
- Usually only in the presence of carbs.
[45:18] Sirtuin production for ketosis.
- BHB inhibits HDAC, which is important before sleep.
[52:07] Keto diet as an anti aging diet.
