[00:00:00] Hey, hey, welcome back to another episode of superhuman radio today is an important show. I know they're all important. If you listen to me, right but know this really is an important show and I'll tell you why. In the past few months. I've been having lots of conversations with guys I grew up. And they're all telling me that they just don't respond to sexual encounters.
[00:00:54] Like they used to some of these guys may have diabetes. Some of them don't some of them are [00:01:00] just older and they noticed that. The little man isn't standing at attention like it used to and these are guys who still have libidos they still have desire to make love to their wives, but they don't feel confident like they used to.
[00:01:21] Because they feel like things are changing and we're so fortunate like, you know, our fathers Generations, they just went down that road and probably got that's why they were grumpy because they wanted to have sex but they couldn't have sex partners are different story when I talk about women right now.
[00:01:39] We just talked about guys and and maybe that's why they became Grumpy Old Men because they felt like that. They got betrayed by their bodies. This is. Longer the case and I'm doing the show for all the guys out there who are suffering in silence because this is not an easy subject to talk about because we attach [00:02:00] our our our manhood to our sexual prowess and when that goes away, we don't feel like men as much anymore.
[00:02:10] So my guest today actually a good friend of mine. Dr. Paul Constanta. How you doing Paul? Hey good girl. How you doing buddy? Wonderful. And then of course Donald Zeno is a Zeno is in. Oh, yeah, it's fine Zen. Oh, okay, and and just so everybody knows Donald is a nurse practitioner who worked with Paul and he specializes in working with men.
[00:02:36] So before we even talk about the different therapies Don, this is a hard subject for guys to even come out and talk about it. I really do think it was cuz I mean it really goes to the basis of us. You know, we try to be. You know stoic about it and it's actually a conversation that it's almost embarrassing to talk about with your doctor and it [00:03:00] and so yeah, it does it and then it just it kind of just snowballs from there.
[00:03:04] And then before you know it, you know you start to have. You know issues with your partner and it just you know, and then like I said, you know, you you get the you start to get depressed and it just it just kind of just rolls from there. Well, so I've actually done shows in the past about sexuality.
[00:03:22] I had dr. Judy. I don't know if you know who she is. She's a world renowned psychiatrist who has a radio show in New York and she came on my show probably about eight years ago talked about a study. They did on monkeys. They put basically diapers on the female monkeys so that the male monkeys could not access them for sex and in two weeks the male monkey started fighting.
[00:03:49] They started exhibiting cannibalistic Tendencies. They started to develop neuroses, you know, they would stand and and and move side-to-side thousands of times like things [00:04:00] that we think about schizophrenic is doing they started to develop all of these really bad symptoms. And as soon as they were allowed access to sex all of that subsided now keep in mind that guys who have erectile dysfunction.
[00:04:14] They still have libidos. So whether the woman is denying them or they just can't function any longer this turns into horrible horrible things. In fact, I just invited a scientist on the show who wrote a book the new rules and she is a sociologist from the UK. Her name is dr. Catherine Hakim and she talks about in her book that she attributes a certain areas of violence to men who are being sex-starved whether it's because they can't function any longer because they're women are not giving them sex.
[00:04:48] But men don't do well when they want to have sex, but something is stopping them. They develop psychological problems. And we see this in the animal kingdom. We see this in humans and we [00:05:00] can't deny it anymore. We can't say. Oh just you know, you're getting older just get used to it. No, we can't say that anymore.
[00:05:06] And we don't have to do e Dr. Paul. There's lots of things we can do now, right? Yeah, 40 percent of men over the age of 40 have some type of erectile dysfunction and almost 70% of men over the age of 70. So it's his taking a toll on society. And I know I want to want to delineate what we're going to talk about today.
[00:05:25] So we're not going to talk about testosterone replacement because let's assume that the guy's testosterone levels and not the issue for a second because we can talk about that, but that doesn't solve the problem. That just gives you more of a libido. It doesn't always give you an erection and the other thing we're not going to discuss on this show today, but we will discuss at a later time.
[00:05:42] I'm lining it up is anxiety as men get older they develop anxiety that plagues them in the bedroom and I my first exposure to this was a guy that you and I both know at the gym. I'm not going to need mention his name, but he had a mild heart attack and he became nervous about having sex and every time he went [00:06:00] in the bedroom with his wife he would worry about having a heart attack while he's having sex in his.
[00:06:03] Wouldn't work anymore. So anxiety plays a role to but we're going to talk about today are actual metabolic functions. The Machinery itself. We're going to talk about making sure the Machinery is working. Right? So if you don't have anxiety and you do have a libido you can actually do what you want to do.
[00:06:21] So we'll let we're going to stick in that and I don't want to get emails from people saying Oh, but you didn't talk about testosterone. Sometimes it's not testosterone. Sometimes the Machinery needs to be repaired. So don. I want to start with you. This is in no particular order but there's lots of things out there that seem to have some clinical relevance when talking about erectile dysfunction.
[00:06:44] I want to start with the one that seems to be joked about the most but is also the most simplistic and we're going to actually talk about this in a later discussion as well. But that is the. Infamous penis pump we see them out there. They're [00:07:00] used as jokes, you know about guys who are lame and they can't get a girl and there but the reality is the penis pump does have a legitimate place in this discussion doesn't.
[00:07:11] Oh, absolutely. I mean it's for those who are familiar with the penis pump is essentially just a hollow tube with a you know, vacuum applied to it whether you use a whether it's battery powered or hand-powered and essentially the man places it over his penis and then it creates a suction and then he and then it pumps up.
[00:07:34] Well that pumping action. Bye-bye by removing the air from that tube. It actually pulls blood and to the man's penis and in what's he so it's essentially he's given himself an erection by by using that pump what happens that is a man can put sa rang the help after he gets the erection. He puts that ring around the [00:08:00] base of his penis and actually traps the blood in his penis.
[00:08:05] So at that point, yeah. The couple can have sex. So it's a lot of what I want to inject something here. No pun intended. Okay, and dr. Pol you may want to speak to this as well. Once I discuss it. So the reason the penis pump is interesting to me is most of the men who have erectile dysfunction today a large majority of them.
[00:08:23] They have metabolic disorders, like type 2 diabetes or pre-diabetes or blood pressure issues. And what happens is the corporate. Whatwhat's the debt spongy 707 OSHA. So when you have angiotensin-converting enzyme pumping out of your body all the time, which is a byproduct of type 2 diabetes that it actually constricts the peripheral vasculature to keep blood in the core of the body.
[00:08:51] The reason it does is because this is part of the fight or flight mechanism when when a saber-toothed tiger was chasing you. Your body said [00:09:00] we really don't need a lot of blood out at the fingertips. We needed in here with a heart can pump and give it to the leg so we can run so angiotensin-converting enzyme was squirted out and all of the non-essential blood flow was restricted kind of closing like closing the vents in your heating system in rooms, you don't use so that the heat is diverted to Rooms you do use the problem with this is that guys who have type 2 diabetes pre-diabetes in these types of problems.
[00:09:26] This is turned on 24/7 365 and guess what one of the non-essential blood flow patterns is into your penis. So when your penis is constantly being constricted and saying and the these chemical produced by the body to actually cause blood not to go into the penis happens 24/7 365. Well, then when it comes time for you to have an erection those blood vessels aren't even big enough to accept blood anymore.
[00:09:53] Is that a fairly? Down but accurate assessment of what's Happening Here. Dr. Pol it is and one of the things [00:10:00] too that should really plan a factor to what those type of diseases in aging. There's also a micro plaquing in these blood vessels so that it's like hardening of your arteries where the blood vessels have a plaque and then they can't expand.
[00:10:14] Interesting very very I didn't realize that so in this case coming back to you Don the penis pump actually causes an overreaching of this spongy tissue thus opening up these blood vessels perhaps for the first time in a decade for these guys and and the other interesting thing was that I found the study that showed that because you are getting a passive.
[00:10:41] You're not forcing the blood in your pulling the blood in there are some benefits to the prostate from a penis pump. Have you seen some of these studies? I hadn't seen all the studies but you but you but you are correct because what you're you know, when you're when you're pulling blood when you know when you're pulling blood into the penis [00:11:00] your your cars in this you.
[00:11:03] You know your cousin the vasodilation which in turn, you know, you're able to better blood flow to your prostate also, because that because by your open your open up that whole channel it's got which is called the the vesicle cement seminal vesicle complex or something like that, which is your prostate.
[00:11:23] It's also the blood vessels that go to it. It's also the plumbing that carries sperm to into the semen and all this other crap into the area. Yeah and. Well, so let's not discount. You know, we made fun of the penis pump for very very long time. But the reality is there are a lots of guys out there who would actually benefit and one other thing.
[00:11:41] I want to throw in here and down feel free to jump in. There's a phenomenon called nocturnal tumescent erections. This is maintenance on your penis and a lot of guys have told me I get amazing erections at night in the middle of the night, but when it comes time to go make love I don't [00:12:00] this actually.
[00:12:01] May fall into the hole anxiety erectile dysfunction category. But the reality is the reality is that when you use that penis pump, they saw in one study a progressive increase in the quality of erections during the nocturnal emission not erection process it and so that's actually something interesting Don isn't.
[00:12:24] I mean, I think it's very interesting but you do you bring up a good point though. And yeah, I know we're not going to talk about on this on this on this program. But I mean stress anxiety and different mental health conditions do affect that and when you're asleep, you know, you're not you're not affected by those.
[00:12:40] Yeah, that would that would make sense. Yeah, I had one guy tell me I get the most amazing erections in the middle of the night but in the morning when I want to make love, I I'm afraid she's gonna say no, you know guys don't we don't respond well to rejection ladies out there. I'm just telling you that just use it as you want.
[00:12:58] But the guys like no in [00:13:00] the morning, I just can't do it and meanwhile in the middle of the night. I had the best direction I've ever had. So there's the penis pump for those who want to explore that they're out there. I looked at price points. This expensive ones is inexpensive ones is hand pump ones his electronic ones.
[00:13:13] So that might be something older guys who. Still able to get an erection but it just isn't as stiff as it used to be a penis pump. Maybe the first stop for you. So now let's move on to some pretty cool Scientific Technology. Let's talk about the acoustic. The availability of acoustic wave now.
[00:13:35] We're seeing more and more being done even advertised on TV where there's an acoustic device. Dr. Pol. What's it called? You, you know the name of it. So it's a low-intensity extracorporeal Shockwave therapy. So Shockwave therapy. It's really interesting how they figure this stuff out as first it was developed to break up kidney stones.
[00:13:56] So if you have a kitty Stone they can use a higher intensity shock wave [00:14:00] and it will actually. Break up the kidney stone. So a low-intensity chokwe what it does is it breaks up the micro plaque inside the blood vessels which stimulates new blood vessels to grow. So this speaks to the micro plaque sofa guy.
[00:14:16] Is is is improving his erections but something still isn't working. He may want to think about the fact that he may have developed some micro plaque in in the vasculature. This would be the next step for him. Right? It also is really neat about it'll work for patients that are responsive to Cialis or Viagra.
[00:14:34] But also the patients that are unresponsive. So basically what you're doing is there's a sheared stress. So it's like a tapping sound this distrust the sound wave. Goes in the acoustic wave goes into the tissue and what it does is it causes an intracellular and extracellular response or the with the body goes through something called neo genesis where it's action and start growing new blood vessels.
[00:14:59] In fact, [00:15:00] you use this on my cforce sheet of seed C4 C5 compression in my spine. In fact, I love the way that machine makes me feel ya Lor your cortisol level their brakes of scar tissue. Yeah. So what so what do you do the guy sits on it? Do you place it where the so the basically a therapist will come in there and they grasped a chef and they they actually tap up and up the sides of the shaft and then they also do the.
[00:15:28] Prostate area as well and this Soundwave treatment takes about 10 to 20 minutes and what it does is as that tapping happens in there. It'll stimulate something called endothelial nitric oxide synthase. And that's basically what Viagra is doing as well interesting but now you but why is this bet so but this is making permanent changes to the vasculature that viagra's not get permanent change to the vascular system.
[00:15:57] Then it also does what it does is cause it'll release [00:16:00] called the vascular endothelial growth factors and those growth factors actually cause the body to heal itself. Wow, that's pretty cool. Now as soon as you say, you know, we put it up and down the shaft and the guys freak out right guys are like, I don't want this guy looking at my penis.
[00:16:17] That's the first thing that comes to mind with me. What do you how do you how do you put Men At Ease about. Well, there's a couple ways everyone you can see do you want a male therapist or a female therapist there at their gloved, you know, and you know, it's it if it's can solve the problem. I think they're more at ease having the erect, you know, not of these having the issue itself.
[00:16:43] Now you raise the you made a statement a second that and now I need to address to there are a lot of guys who don't respond to see Alice. Or Viagra and that's because the blood can't even get into the penis in order to even release the nitric oxide in the [00:17:00] first place. These guys probably do have vasculature issues in other parts of their bodies as well.
[00:17:06] So this should be no surprise to them, right? Yes. So basically you're regrowing the blood supply in that area plus with the therapy itself. I mean, I'm a person can experience an erection while they're doing the therapy and that's just the. Going back into the area how many of these sessions these 20-minute sessions.
[00:17:27] Do you need to start to really see things happening when you're by yourself or with your loved one is starting a series of six sessions and then usually from the 2nd to the 4th session, you will see you know changes you'll you know, you'll see changes such as you know easier arousal or you know, You know, no pills like you won't need your Viagra anymore.
[00:17:55] So what so what I actually have to call a guy about this now that I just thought about because [00:18:00] there's a guy who called me not too long ago. I was talking about this. I'll tell you more about that when we talk about calves reject a little bit later, so. How much is it usually cost? Because there's a lot of clinics popping up now trying to address erectile dysfunction in Men in different ways to give men their their their sexual lives back.
[00:18:18] How much will they pay which arranged for one session and then you're saying you need about six to get started. It's anywhere from three to six hundred dollars a session depending what Clinic. Does insurance pay for that? I got a feeling no, no insurance does not cover. It doesn't that suck. I know pay for it to get cut off but they won't pay for it to fix it.
[00:18:41] Yeah, it's just terrible. It's terrible. I look I want to take a quick commercial break when we come back. We're going to talk a lot more about different Therapies. [00:20:50] How does it work and for guys out there who maybe think that that's the only alternative for. Well, I mean Kappa Jack is you know, and [00:21:00] that's the the trade name for the medicine is actually a profit bill and it's a they described it as a 2d e 2 inhibitor prostaglandin 2D inhibited prostaglandin.
[00:21:15] Yes. And and essentially what it does is it's a it's a vasodilators and it's a smooth muscle relaxer. So what the man does is he uses a fine needle and it's a they ejected into the base of the penis and what the medication does it'll usually takes effect between, you know, five to twenty minutes and it just because it's a there's died later in a smooth muscle relaxer.
[00:21:41] It just allows blood. To rush into the penis at that point that the individual get an erection now will this dr. Paul will this work even if a guy has plaque buildup and he's has and he has vascular tissues in the the spongy tissue in the penis it [00:22:00] probably not or may work. So really going back to Don so really this is a is an alternative for a guy who is getting some erections.
[00:22:13] They're just not as hard as they used to be and he just wants a really hard erection. This is good for him, right? That's correct, you know and before you before the individual can is a candidate for that, you know, we have to rule out certain medical conditions. So like what an individual that has sickle cell anemia or sickle cell trait if you've got a history of leukemias or multiple multiple myeloma.
[00:22:41] Or even if you have a deformed penis shape there is there is actually a condition called Peyronie's disease where the penis is curved Bill Clinton Bill Clinton was famous because when the discussion came out about his sexual exploits the woman actually described his penis and the newscaster.
[00:22:57] Apparently the president suffers from Peyronie's [00:23:00] disease. So he became FEMA's a famous because of that right? Exactly. Sure. So finally the last few don't want if if the individual has a penile implant there. They're not a candidate for for this type of therapy. So I'm going to ask you about penile implants and a little bit because I did want to get some clarification.
[00:23:19] I've never really understood what they do. But okay, so, dr. Dr. Pol. Did you tell me that when a guy uses cabbage act you have to inject yourself to get the erection, but then there's something you have to inject again in order to defeat the erection. That was a misunderstanding. You have an erection that lasts over 2 to 3 hours.
[00:23:39] You better had to your doctor. Yeah. Yeah, exactly. Yeah, and that's why those porn stars are going right after they had. Tofu, after four hours of videoing. But anyway, so that's why a lot of these new erectile dysfunction clinics that they advertise that if the you'll know right away in the office if it works and if it doesn't work, there's nothing because they're [00:24:00] going to give you a shot of this and it's either going to work or not work and you're going to be happy and you're going to go home because it's so spontaneous.
[00:24:07] What is it about prostaglandin? So prostaglandins are produced as part of the. / inflammatory response right when you take a cyclooxygenase inhibitor and NSAID doesn't that also affect prostaglandins and and why is it why do what do prostaglandins do that? Make the penis not functioned on? Well, I'm here prostaglandin is just a group of like hormone-like substances.
[00:24:34] And what they do is they're actually they're involved in contraction and relaxed and smooth muscle. They also do they dilate and constrict blood vessels. They are involved in controlling your blood pressure and modulate inflammation, you know, essentially what a prostaglandin does is. One of the things that does is it's released by the blood of the walls of our blood vessels in response to an infection or inflammation it actually [00:25:00] it's actually the prostaglandin E2 is actually what causes fever.
[00:25:04] So in reality guys who respond well to the cavern ject injection most likely have a metabolic disorder because inflammation is is is elevated in their bodies. Would you would you say that's a true statement? I went to the true statement. I mean it's certainly and and then you know, I mean there's a lot of causes to Edie, you know.
[00:25:28] Certainly, you know the guy we eat. I mean we are we are our diet has caused us to have a lot of inflammation and when that happens, you know that these are the byproducts of this this this inflammation as we get coronary disease and then you know with the corner disease also, then you start to have erectile dysfunction.
[00:25:46] So it's just a it's just a vicious circle and there's a lot of foods that we normally eat, you know, gluten dairy soy are all. World inflammatory Ryan type [00:26:00] substances, right? So I first heard about the penile implant when Flip Wilson the comedian from the 60s had a penile implant put in and I was like, what is that?
[00:26:11] I I envisioned him having one of those little valves like you see on a beach ball where somebody had to blow his penis up and it what is it? What exactly is a penile implant? How does it function? What does it do? Will you actually hit the nail on the head? I mean what you were envisioning is actually one type of implant.
[00:26:29] It's it involves. It's surgically placing devices in both sides of the penis and their implants and they're either inflatable like what you just described or they're malleable like they're bent bendable rods. So the inflatable device is allow you to control when and how long you had the erection so you actually do have like a little.
[00:26:47] Well pump that you squeeze and you get it actually filled with air and causes the the gentleman ever erection the other type of rods, excuse me is it's actually [00:27:00] it's actually bendable, you know, so when he bends the penis up when you want to have an erection and. Yeah, Ben's it down or to the side when when he's not still doesn't intersect.
[00:27:12] So doesn't this may be an I've now you've questioned but doesn't the penis have to be erect like how I like doesn't the activation of the penis that makes it a wrecked have to be part of the orgasm process or can guys climax, even though they're being assisted in having a penis that sticks out, you know hard to call it an erection.
[00:27:30] But like with a peanut penal implant. Well, it's the. You know with with it, you're right what the malleable I would think they were probably it's it is harder to come to come to climb at right but I think you know, you know, if you're you know that you're still like we like you talked about early you still have libido and you're still having those feelings.
[00:27:52] So I was it menu. There are men that will still have a soul, you know, Soul ejaculate, but you're absolutely [00:28:00] right, you know if it's. If you're not sexually stimulated then you're not going to that portion is not going to contract and cause you to ejaculate. Dr. Paul what about pulse electromagnetic fields?
[00:28:12] We're seeing these used more and more in different types of therapies. And there seems to be some interest in using pmf to Aid and erectile dysfunction of all of these therapies we talked about so far. Is that a player or is that just not really that good? I wouldn't say it's not that a player because yet every therapy were talking about has one thing that we're trying to do which is manipulate or increase the oxygen utilization or or putting oxygen to the neovascular beds of the body.
[00:28:41] So anytime you can get an increase in circulation to that area. It can help us one thing interesting about PMS. It also knowledge is more blood to the area but can also decrease hypertrophic, you know prostitute. All right. Yeah, but I prosthetic hypertrophy. Yeah, exactly. Yeah, [00:29:00] so I think it's a you know possible good player in there.
[00:29:04] I mean, it's you're basically sitting on a high voltage ring and it puts a shock that actually kind of like electrical. He's a. The prostate where as it goes through the body, whereas the acoustic wave therapy is a sound wave going in the body, but both of them are designed to get more blood flow in the area the differences with the acoustic wave therapy.
[00:29:28] It actually regrows blood vessels. And actually heals the therapy kills the area From the Inside Out whereas the pmf can get more fluid, you know more increased blood supply there, but I don't think it will maybe even you know, my opinion is I don't think it's as effective as let's say the acoustic wave therapy.

And apparently there's some benefits to it when using it for erectile dysfunction. Is that correct? That is [00:33:00] correct. Actually, you know, it's the it's a.
[00:33:04] Trent it really is. It's using multiple multiple Medical Specialties. I know they use it in dermatology opthamology Cardiology plastic surgery. I mean, we've heard of women getting the or anyone people getting the vampire facial. That's what exactly what that is are using, you know PRP or essentially it's called platelet-rich plasma.
[00:33:27] So just so everybody understands how this works and we've actually done shows on PRP for sports injuries. They draw blood they centrifuge it. They separate the plot the the the platelets and then they inject them back into the area that they want fixed now platelets are there for this purpose, but what you're basically doing is instead of waiting for the body to willy-nilly send the platelets to the injury.
